Then mention your website Url name ( Domain Name)

Billing information should be same which is you already fill in your account

If you have an single account then choose single account. If you are using MCC ( My Client Center ) 

then choose it. 

Are you advertising your own business if yes then choose yes otherwise choose no

Who pays for the adword account your client or you.

Select your Adword payment option 

Date of your last payment. You can find your last payment date from billing setting tab

Mention your country 

Fill a description about your business & also additional information
